When ordering uniforms, please do the following:

1. Please download the Electronic Uniform Requisition Form, fill it out and save it for your records.
2. Email it to uniforms@medic911.com.

NOTE THE FOLLOWING WHEN PLACING ORDER:

• All boot requests should be sent to this email address with a specific style, size, and item numbers.
• You do not need prior authorization to be fitted for uniforms. However, we ask that you present your Medic ID badge at the time of the fitting.
• If you are unsure of your current uniform sizes, style, etc., please visit Anderson Uniform Co. Inc. at 5630 NC Hwy 49, Harrisburg NC to be fitted for different options. Please see the SOG.
